HOKKORIについて

障がい者グループホームとは、自立を目的として成長をサポートする施設です。
一般的には、高校を卒業する18歳から入居が可能になります。
つまり、グループホームが足りなければ、デイサービス卒業後に行き場がなくなるということです。

私たち障がい福祉の専門家が、『HOKKORI』の立ち上げによって、
・障がい者さんも人生を、グループホームを自由に選択できる世界に！
・障がい者さんが自立できる世界に！
・保護者の方も安心できる世界に！
を実現します。

【サービス内容】
○生活支援　食事の提供、身体保清、金銭管理等。
○日中活動支援　就労先などの連絡、引渡、買い物、外出、季節の模し物など。
○社会的支援　生活相談、対人関係、書類管理、各種手続など。
○健康管理の支援　服薬管理、通院支援、医療機関との連携など。
